Request Forms
The forms available on this page are in a PDF (portable data file) format
and can be read and filled out with Adobe
Acrobat Reader 4.0 or later which you may download from this page. Take all of the defaults
when installing. After installation is complete you need only click on the PDF form files that you
need to use. It will take up to 45 seconds to download the form, depending on the speed of your
connection.
Tips for filling out the forms:
1 Use the tab key to advance to the next
field.
2 Social Security number can be entered
with or without dashes.
3 Telephone number can be entered with
or without dashes.
4 All dates should be entered in MM/DD/YYYY
format.
5 To start over, press the "RESET" key
at the bottom of the form.
This will clear the form of all entries that have been made.
6 When you have completed the form, print
it, sign it where appropriate
and either mail or fax the form as indicated at the bottom of the page.
Residential
Service Suspension Request Form
Customers who need to discontinue their garbage and trash service for an extended
period of time can do this by completing this form. Service must be suspended for a minimum of
three months; and upon receipt of this form, Sanitation officials will collect the automated cart
assigned to the residence and hold it until service is resumed.

Off-Street Collection Request Form
The Off-Street Residential Collection Service is designed to assist individuals
who are impaired by a physical disability which makes it difficult or impossible to transport
their automated container to the road for collection. Individuals applying for this free service
must possess a verifiable physical disability (in the judgment of Sanitation Division officials)
and must either live alone or have no one living in the same household who is physically able
to transport the cart. If approved, customers may leave their cart at a convenient place next
to their house, and workers will remove, empty, and return the cart on collection days.
